Donation Request Form

Please enter your information in the following fields below so that we can consider your request. A brief one paragraph description of your event is appreciated. Please do not copy and paste your brochure.

If we do provide you with a donation, we will request the submission and use of your organization's logo on our website if we do not already have it. This not only serves to acknowledge our participation in your fund raising efforts, but also provides an additional opportunity for visitors to our website to learn more about your organization.

Any donation that includes a display will require a deposit that will be refunded when the display is returned. Not all donations require the use of a display so please ask if you have any questions.

Please make sure that your group or organization is a school, nonprofit, or charitable organization. We try to satisfy as many requests as possible but cannot make any guarantees.

It’s important to:

Respond promptly to our email confirmation so that we can place you on our recipient’s list. You will be given further instructions based on the specifics of your event.

All donations are granted on a first come basis.

You can apply anytime but donations cannot be picked up more than 10 days before your event.

All decisions are entirely the discretion of NWArestaurants.com.
